About me

I’m Sara Nelson (she/her), a Nurse Practitioner and Primary Care Provider practicing integrative medicine—an approach that blends conventional and natural therapies to create individualized care. I use pharmaceuticals, herbal medicine, nutrition, movement, and intentional rest to support the body’s innate healing capacity. I emphasize lifestyle practices that foster long-term wellness and reduce harmful environmental exposures. My philosophy honors the strengths and limitations of all healing modalities, focusing on what works best for each unique individual.

With 15 years of experience in healthcare, I’ve cared for patients with complex chronic illness, liver disease, gastrointestinal disorders, hematologic conditions, cancer, and stroke. I’m a generalist at heart, and I love the variety that primary care offers. Every patient brings a unique story, and I’m deeply curious about how the body’s systems interact. Whether someone arrives with a straightforward concern or a complex constellation of symptoms, I enjoy exploring holistically to uncover what supports healing. My clinical interests include brain health through whole-body healing, gastrointestinal wellness, and autoimmune conditions. In primary care, I help patients navigate the healthcare system and build the holistic care team needed for support and empowerment. If you haven’t had a good experience with the medical field, I’m a good person to see—I ground my care in listening, learning, and partnership.

I celebrate the richness of varied human experiences and provide inclusive, affirming care for patients in nontraditional relationship dynamics, BIPOC communities, and LGBTQIA+ individuals. My practice is a welcoming space where every identity is respected and validated.

Outside of clinical work, I enjoy exploring global cuisines, sipping decaf in Portland’s coffee shops, and staying active. I share my life with two partners in a polyamorous relationship. My adventurous spirit finds expression in rock climbing, ski mountaineering, canyoneering, and hiking—always seeking to stay wild and connected to nature.
